The Role
This is an opportunity for a skilled and motivated Certified Occupational Therapy Assistant to join a close-knit rehabilitation team at a busy, full-service care facility in Fremont, CA. Working directly alongside licensed Occupational Therapists, you will play a hands-on role in helping patients rebuild functional abilities, regain independence, and improve their overall well-being. This contract position follows a consistent Monday through Friday schedule with no weekend rotation — offering stability and work-life balance in an active clinical setting.

What You Will Do
As a key member of the therapy team, you will carry out individualized treatment plans developed by supervising Occupational Therapists, leading therapy sessions and therapeutic activities tailored to each patient's goals and functional needs. You will closely monitor patient progress, identify changes in condition or ability, and communicate observations promptly to the supervising OT to ensure care plans remain responsive and effective.
Patient and family education is a meaningful part of this role — you will guide individuals through exercises, adaptive techniques, and daily living strategies designed to support long-term independence beyond the facility. Accurate, timely clinical documentation is essential, and you will maintain thorough records of treatment sessions and patient progress within the facility's EMR system.
Collaboration is central to how this team operates. You will work alongside nurses, therapists, and other healthcare professionals to deliver coordinated, whole-person care. You will also help maintain a safe, organized therapy environment and stay current with evolving best practices in occupational therapy to bring the highest standard of care to every patient interaction.

What We Are Looking For
• Active COTA license or state certification
• 2+ years of experience in a healthcare setting
• Proficiency in EMR systems and clinical documentation
• Demonstrated ability to implement OT treatment plans
• Strong patient and family education skills
• Solid knowledge of occupational therapy principles and techniques
• Ability to perform physically demanding tasks (lifting, transferring, positioning)
• Clear, effective communication and interpersonal skills
• Comfortable working both independently and within a team
The ideal candidate brings at least two years of hands-on clinical experience and a thorough grounding in occupational therapy practice — from treatment plan execution to progress tracking and documentation. You are someone who takes pride in patient-centered care, communicates clearly across disciplines, and can manage the physical demands of an active rehabilitation environment. A current, valid COTA license and familiarity with electronic medical records are essential from day one.

Nice to Have
• Experience working in a skilled nursing facility (SNF)
• Familiarity with SNF regulatory and documentation standards
• Exposure to complex, medically involved patient populations
Candidates with a skilled nursing facility background will find themselves especially well-prepared for this environment. SNF experience signals comfort with higher-acuity patients, familiarity with the pace and documentation expectations of post-acute care, and an understanding of the interdisciplinary team dynamics that define this setting. While not required, this background can make for a faster, smoother transition into the role.
