About This Role
Physical Therapist Assistant (PTA) — Contract | Fremont, CA
About the Company
A leading geriatric rehabilitation company dedicated to restoring independence and quality of life for older adults, this organization operates a well-established skilled nursing facility in the heart of the Bay Area. Their multidisciplinary care teams are known for a patient-first philosophy and a collaborative, supportive work environment. This is a place where clinical excellence and genuine compassion go hand in hand.
The Role
This is an exciting contract opportunity for a skilled and motivated Physical Therapist Assistant to join an experienced rehabilitation team serving geriatric patients at a busy skilled nursing facility in Fremont, CA. Working Monday through Friday on-site, you will partner closely with licensed physical therapists to deliver hands-on, individualized care that makes a meaningful difference in patients' daily lives. If you are passionate about geriatric rehabilitation and thrive in a structured, team-oriented clinical setting, this role is built for you.
What You Will Do
• Carry out individualized treatment plans developed by supervising physical therapists, targeting improvements in patient mobility, strength, balance, and functional independence
• Track patient progress across sessions, adjusting observations and communicating meaningful updates to the physical therapy team and broader care staff
• Maintain thorough, timely documentation of all patient interactions and treatment outcomes within the facility's electronic medical records system
• Provide clear, practical guidance to patients and their families on home exercise programs, injury prevention strategies, and safe movement techniques
• Work alongside therapists, nurses, and other healthcare professionals to coordinate care and support the best possible rehabilitation outcomes
• Uphold all applicable facility policies, state licensing requirements, and federal regulations governing physical therapy practice
What We Are Looking For
• Active PTA state license in good standing
• Minimum 1 year of clinical experience in physical therapy
• Demonstrated ability to implement and progress treatment plans
• Proficiency with electronic medical records platforms
• Strong patient and family education skills
• Solid grounding in physical therapy principles and techniques
• Effective communicator with a collaborative, team-first mindset
• Compassionate, patient-centered approach to geriatric care
The ideal candidate brings at least one year of hands-on clinical experience and is comfortable working within a supervised, team-based rehabilitation model. You should be confident translating a physical therapist's clinical plan into effective, responsive treatment sessions while keeping documentation accurate and current. A genuine interest in working with older adults — and the patience and empathy that requires — is essential to succeeding in this role.
Nice to Have
• 1–2 years of experience in a geriatric-specific clinical setting
• Prior experience working in a skilled nursing facility (SNF) environment
• Familiarity with SNF regulatory and documentation standards
Candidates who have spent time in skilled nursing or long-term care environments will find themselves well-prepared for the pace and patient population here. Experience navigating the unique clinical and administrative demands of an SNF — from complex comorbidities to compliance documentation — is a genuine advantage. That said, motivated clinicians with strong foundational skills and a passion for geriatric care are strongly encouraged to apply.
