About This Role

Physical Therapist (PT) — York, PA About the Company A leading skilled nursing and rehabilitation company dedicated to restoring function and improving quality of life for the patients in its care. With a strong clinical foundation and a collaborative, team-driven culture, this organization delivers individualized rehabilitation services that make a meaningful difference every day. Their York, Pennsylvania facility is a well-established community resource committed to excellence in post-acute care. The Role An experienced Physical Therapist is needed to join an active rehabilitation team at a skilled nursing facility in York, PA. This is a full-time, on-site position designed for a clinician who brings both strong hands-on skills and a genuine passion for helping patients regain independence. You will carry a meaningful caseload, work alongside a dedicated interdisciplinary team, and have the opportunity to see the direct impact of your clinical work in patients' daily lives. What You Will Do Perform thorough physical therapy evaluations, reviewing medical histories and identifying each patient's functional limitations and rehabilitation priorities. From there, you will design and carry out individualized treatment plans that draw on therapeutic exercise, mobility and gait training, strengthening programs, and balance interventions tailored to each person's goals and diagnosis. You will track patient progress consistently, adjusting treatment approaches as clinical needs evolve to keep recovery on the best possible trajectory. Accurate, timely documentation of evaluations, session notes, and discharge planning recommendations will be a core part of your daily workflow. Beyond direct patient care, you will partner closely with physicians, nursing staff, occupational therapists, speech-language pathologists, and other members of the care team to ensure coordinated, patient-centered treatment. You will also take time to educate patients and their families on safe movement techniques, home exercise programs, and strategies to support continued progress outside of formal therapy sessions. Staying current with evidence-based rehabilitation practices and contributing to a culture of clinical excellence will be an ongoing expectation of the role. What We Are Looking For • Active Pennsylvania Physical Therapist license • 2+ years of experience as a licensed Physical Therapist • Proficiency in PT evaluation and clinical assessment • Individualized treatment planning and goal-setting skills • Therapeutic exercise prescription and program design • Mobility and gait training expertise • Accurate, timely clinical documentation • Strong interdisciplinary team collaboration • Availability for a five-day schedule including weekend rotation • Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program The ideal candidate is a confident, well-rounded clinician who has moved beyond the learning curve of early practice and is ready to work with a complex, medically diverse patient population. You bring the ability to assess patients thoroughly, build realistic and motivating treatment plans, and communicate clearly with both patients and colleagues across disciplines. Your documentation is precise and completed on time, and you take pride in delivering care that is both evidence-based and genuinely person-centered. This role calls for someone who can work with a degree of autonomy while remaining a dependable, engaged member of a larger care team. Nice to Have • Experience in a skilled nursing facility (SNF) setting • Background in geriatric care and age-related conditions • Prior work in a post-acute or inpatient rehabilitation environment Candidates who have worked in skilled nursing or geriatric rehabilitation settings will find themselves especially well-prepared for the pace and patient mix of this role. Familiarity with the regulatory environment and documentation standards common to SNF settings is a genuine advantage, as is comfort working with older adults navigating complex recovery journeys. That said, a strong clinician with solid acute or outpatient rehabilitation experience and a clear interest in this population is equally encouraged to apply. What We Offer This is a full-time, on-site position based in York, PA, offering a consistent daytime schedule and a structured clinical environment where your work has visible, daily impact. You will join a collaborative rehabilitation team that values professional input and supports ongoing clinical development. The organization offers opportunities to grow your skills in post-acute and geriatric care, with access to a team of experienced clinicians across therapy disciplines. For the right Physical Therapist, this is a role where meaningful patient outcomes and long-term professional growth go hand in hand.
